DentServ is a known provider of dental services for long-term care facilities. rehabilitation centers and community-based clinics throughout New York State. We provide dental care and services to individuals with special needs by establishing operatories and providing treatment sessions in assigned facilities and centers.

If you are interested in an opportunity to work with individuals needing dental care, review the enclosed application package and complete the following forms:

• DentServ Dental Services PC Application Form
• Two (2) Reference Request Forms
• Consent and Release
• Authorization Form
• Copies of any certificates, education and/or training applicable to the position

Hygienists must submit and include:

• A copy of current NYS registration
• A copy of current malpractice insurance (must include carrier name, type and/or extent of coverage and expiration date)

Please note that any non-licensed person providing direct care or supervision to patients or residents in long term health care or residential health care facilities or programs must complete and pass the New York State – Department of Health Criminal History Record Check (CHRC). This will require consent, completion of the NYS-DOH form, and submission of fingerprints. Processing and paperwork will be completed at the facility(s) in which staff are assigned.
